SSS Loan Restructuring Program Download Application Form and INSTRUCTIONS

For details if you qualify or not for the SSS loan condonation program, check this post for the SSS loan restructuring program Eligibility Requirements.

Follow this application instruction when you apply for the SSS LRP Program.

1. Secure updated Statement of Loan Balances for Loan Restructuring Program (2 copies) from the nearest SSS branch or foreign office prior to submission of the SSS LRP Application Form.

2. Accomplish an Affidavit of Residency attesting residence / work address in a covered calamity area when the calamity / disaster happened (except for member-borrower with Calamity Loan or Salary Loan Early Renewal Program (SLERP) and Death Benefit filer).

3. Fill out the SSS Loan Restructuring Application Form, you can download this form online or get one from from an SSS branch.

4. Always indicate “N/A” or “Not Applicable, if the required data is not applicable.

5 Always affix initials on all erasures / alterations on this form.

6. If member-borrower is filing the LRP Application form, present original copy of valid identification cards / documents. Check out the the “List of Filer’s Valid Identification (ID) Cards / Documents”.

7. If a representative is filing this form, submit the following:

a. Special Power of Attorney (SPA) – original copy

  Check out the SAMPLE SPA.

b. Photocopies of two valid identification cards each of the member-borrower and the filer, at leat one (1) with photo and both with signature present original. Check out the “List of Filer’s Valid Identification (ID) Cards / Documents”.

8. Submit the SSS Loan Restructuring Application form to the nearest SS branch or foreign office with the Statement of Loan Balances for Loan Restructuring Program signed in the “Conforme” portion and the notarized Affidavit of Residency (if applicable).

9. For Death, Total Disability, or Retirement (DDR) benefit filer who will avail of the Loan Restructing Program of SSS, submit the LRP Application form and Statement of Loan Balances for Loan Restructing Program together with the DDR claim application

Download the SSS Loan Restructuring application form. You can print it, fill-it up before you visit the SSS branch nearest you.

If you don’t have a printer, you can get the form from any SSS branch.

Update: There is a NEW SSS LRP for 2018 program!!!

Make sure your documents and other requirements are complete when you submit, so that processing of your SSS Loan Restructuring application will also be fast.

    • Hello Leticia, on your query, it would be best to visit a SSS branch nearest your home so they can answer all your loan queries.

  3. Hi. How do I go about payment of first installment for approved SSS LRP?
    Do I just use the usual SSS Form ML-1?

    • Hi Aleta, per consultation with an SSS personnel, there is a specific loan payment form for restructured loans (you will not use the SSS Form ML-1). You can get the special payment form at any SSS branch.

  4. hi ask ko lang po kung qualified ba ako sa LRP?2007 pa yung utang ko at hindi nabayaran at sa ngayon ay may sakit na lopus na ako?person with disability.

    • Hi Ms Ellen, are you a resident of an area that was previously declared to be under ‘calamity’ by the government, if yes po, puede po kayo mag-apply.

      Also, Ma’am check niyo po etong PCSO Requesting for Medical Assistance in case di pa kayo nakahingi ng tulong sa PCSO, nawa po’y matulungan kayo sa sakit niyo ngayon.

    I am Nejima A. Delector OFW and a bonafide member of SSS. I had my first loan before but due to suddent lost of job, i was not able to fully paid my first loan. For now i don’t know if how much is the current balance. With this, i would like to restructure my loan or request for condonation because i want to have my house be renovated and improved. Please advise me through my email add here below.

    • Hi Nejima, just ready your requirements and then go to SSS to file loan reconstructing. Make sure magawa mo before the April 2017 deadline of the loan condonation program.

  6. I have previous salary loan in sss but i never paid even once. Ask k lng if pwede ako magapply for sss loan restructuring even hnd under ng state of calamity ang place namin?

      • Hi, I would like to know which banks are accredited to pay the restructured loan. Ang advise ng SSS branch namin sa Sta. Maria Bulacan pwede daw sa BDO pero when they are about to pay sa malapit na branch sa amin hindi daw po sila tumatanggap. Thank you.

        • Hi Anna, the last time we checked direct with an SSS personnel ang sabi sa amin is that ang payments for approved restructured loans can only be paid in a SSS branch kasi a special payment form will be used (form that is exclusive for restructured loans).

  8. Hi ask ko lang i already apply last year of lrp pwede pa ba ako mag re apply ngaun kasi na dis continue ko ung pagbabayad?
