For those who can’t personally process their application for the loan penalty condonation program of the SSS or the Loan Restructuring Program, you need to present a Special Power of Attorney, or SPA.
In the SPA you will indicate who your Attorney-in-fact, and to authorized him or her to apply for Loan Restructuring in your behalf, as well as “to request, make, sign, execute, and deliver all the necessary documents” in relation to your application in SSS, among others.

You as the member-borrower, and your Attorney-in-fact designated in the SPA needs to sign the Special Power of Attorney before a lawyer. Notarization of the SPA is required.
