How to Apply for PCSO Medical Assistance – REQUIREMENTS

All Filipinos who are hospitalized or receiving medical treatment can apply for the PCSO Individual Medical Assistance Program (IMAP). You don’t have to be an indigent patient to get approval; you don’t have to personally know anyone serving a government position to become a PCSO medical financial assistance beneficiary.

Any Filipino can apply to become a recipient of PCSO Medical Charity program as long as the patient fulfills any of the following criteria:

  1. You are confined in any health facility.
  2. You are receiving health care management as outpatient.
  3. You are seeking health care management in another country, because there is no existing health facility within the Philippines that can provide the procedure that your illness demands.

Apply to Become a Beneficiary of PCSO Medical Financial Assistance

Getting sick and being hospitalized is a big burden especially when it comes to the financial aspect of becoming ill. Even more so if you or your family member who is ill doesn’t have any HMO coverage or health insurance, or worst doesn’t have any money to pay the hospitalization bill, the needed surgery, the needed medicine.

That is why the PCSO or the Philippine Charity Sweepstakes Charity Program is a big help for those who need kaagapay sa pagpapagaling. Do not hesitate to apply at PCSO. If another family member is sick, you can help process the PCSO application for medical assistance.

Requirements to Apply PCSO Medical Assistance

If you meet the criteria above, you now need to prepare all the needed requirements, the documents and papers that you need to submit to the Philippine Charity Sweepstake Office to avail the Medical Assistance.

Once you get approval like for hospital confinement there will be a PCSO guarantee letter that is issued to the hospital or partner health facility, detailing that PCSO will assume the obligation to pay a specific amount due of the patient to the health provider.

Unfortunately, PCSO usually will not be able to shoulder the full amount due of the patient-beneficiary for the treatment received. This is because the Individual Medical Assistance Program or IMAP deals with thousands of patients every day all over the Philippines. All these patients need financial assistance and PCSO gives as much help as they can to everyone who applies for medical financial assistance within the budget allocation for the program. But still, financial help is a welcome thing no matter the amount. But of course we pray and hope that you will be granted and approved for a big amount from PCSO.how to apply pcso medical assistance program

Here are the list of documentary requirements when you apply for PCSO medical assistance:

  1. Duly accomplish PCSO IMAP Application Form – if the hospital has its own PCSO desk you can get it from there. If none, you need to go to the nearest office of PCSO.
  2. Original / Certified True Copy of the Updated Clinical Abstract duly signed by the attending physician with License Number. If you are requesting for free wheelchair, you only need to present medical certification.
  3. Valid ID of patient and representative – bring any government issued identification card like Passport, UMID, Driver’s License, GSIS eCard, Senior Citizen’s ID, DSWD – 4P’s ID, Voter’s ID, Postal ID, PRC ID.

If you do not have any government issued ID, you can also present company ID or student ID.

Additional (as applicable) – *Authorization Letter from patient is needed in cases where there is no immediate relative available for interview.

All cases applied for in PCSO requires the documents listed 1,2,3 above. Then aside from the above, there are additional requirements which are case specific.

A. For Confinement

  • Original copy of the Statement of Account / Hospital Bill duly signed by the Billing Officer / Credit Supervisor”
  • Endorsement from the Medical Social Services of the health facility (for charity / service patients).

B. For Dialysis

  • Endorsement / Certificate of Acceptance of PCSO Guarantee Letter

Note: This is not required in the cases of health facilities where there is a PCSO desk.

  • Official quotation from service provider
  • Relevant laboratory results
  • Certification from Dialysis Center

C. For Medicines including Chemotherapy:

  • Relevant laboratory results
  • Photocopy of Histopath / Biopsy Report
  • Original prescription duly signed by physician with license number
  • Progness Notes from the doctor / oncologist.

D. For Radiotherapy

  • Endorsement / Certificate of Acceptance of PCSO Guarantee Letter

Note: This is not required in the cases of health facilities where there is a PCSO desk.

  • Official quotation from service provider

E. Surgical Supplies

  • Official quotation with breakdown of expenses.

F. Implant

  • Official sealed quotation from 3 suppliers
  • Copy of physician’s order with implant specification
  • Police Report – for medico-legal cases

G. Laboratory / Diagnostic Procedures

  • Official quotation from service provider
  • Laboratory request duly signed by physician

I. Assistive Devices – Hearing Aid (Bone Anchored Hearing Aid (BAHA))

  • Audiological Evaluation Report signed by a License Audiologist

J. Assistive Devices – Wheelchair

  • Progress note from the doctor

K. Assistive Devices – Prosthesis (Arm, Leg, Eye)

M. Non and Minimally Invasive Procedures (Laparoscopic Surgery, Endoscopic procedure, ESWL, etc)

  • Endorsement / Certificate of Acceptance of PCSO Guarantee Letter

Note: This is not required in the cases of health facilities where there is a PCSO desk.

  • Relevant laboratory result

N. Transplant (Kidney, Liver, etc)

  • Relevant laboratory result
  • Cross – matching result
  • Certification of patient’s inclusion in transplant program from authorized representative of NKTI, if applicable.
  • Relevant PhilHealth tracking number certification

O. Rehabilitative Therapy (Physical, Occupational, Speech)

  • Official quotation from service provider

You can also apply at PCSO for Cardio procedures like Pacemaker surgery, Congenital Heart Surgery, Coronary Artery Bypass, Aneurysm Surgery, Peripheral bypass surgery, Angioplasty, also included are cardio diagnostic procedures like Coronary Angiogram and Cardiac Cauterization.

Bring all the needed documents when you apply at PCSO office for medical financial assistance.

Again, please take note that every applicant must submit requirements number 1, 2, and 3 above, plus the case-specific requirement(s). Also, if the patient doesn’t have any immediate relative that can be interviewed by the PCSO, the patient must also sign an authorization letter indicating the name of the non-relative who will be interviewed for the patient’s case.

Check the list of PCSO ASAP partner hospitals, if the hospital has an ASAP desk there is no need to go to a branch of PCSO to file for medical assistance.

    My husband was hospitalized last Feb. 29,2016. Can I still ask help from PCSO? How about his monthly maintenance? He is also under treatment of Psoriasis.

    Good day po.. I have cancer. Can I apply for assistance if to be used for alternative procedures like hollistic/ homeopathic healing? They charge as high as Hospitals also. I prefer the alternative procedures but no money for that because we have many children.. Thank you very much for answers.

              Ma’am dapat po sa mismong officer kayo ng PCSO nagtanong at lumapit, hindi lang po sa guard. Ang guard po iba po function nila, di nila alam kalahatan ng programa ng PCSO.

    Sana isa ako sa matulungan ng PCSO..i suffered POLICYSTIC LIVER DESEASE at wala po ako kakayahan para sa operation at pag maintain ng mga gamot ko..i wish na isa ako sa mga libo libo patient na matulungan ng PCSO. Ngaun pa lang nagpapasalamat NA PO AKO SA ATING MABUTI AT MAKATAO si PRES. RODY DUTERTE sa knya bago programa ito sa PCSO at sa mga kumakatwan nito. Salamat sa Diyos! At MABUHAY PO KYO!!!

    Do uou require early application for ID in this peso medical assistance program while we are not yet confined? What about patients who are under regular periodic medical check up and those patients required to take maintenance medicines, lab tests…are prof feed and labtest costs covered? I am a senior citizen. Thank you.

    Hi Good Day, ask ko lang po may maintenance na mother ko, she is diagnosed parkinson Desease , may maintenace na siyang gamot, pede ba kaming matulungan sa maintenance the whole, kasi last year pinipili lang nila ang gamot na ibibigay, kung alin yung lessen ang price, yung mahal na gamot di nila binibigay, ang now may bedsore siya, ang mahal ng RTD wound care dressing? Php780 one piece that is for 2 days only, eh matagal po ang bedsore bago gumaling. Please help us for the maintenance medecine kasi di na kumakain ng solid food mother ko mostly blended foods and milk, pinapadaan na lang sa hose na nakakabit sa nose nya,Heto mga medecine nya requip 8mg, parkemet 275mg, akeniton 2mg, regeron E, gabix 100mg,candes16mg, flumucil 600mg. thanks

      Hi Ma’am we recommend po that you go to the nearest PCSO branch para po ma-relay niyo yung case history ng mother niyo and makarequest ng tulog direct sa PCSO. Or call po kayo sa telephone numbers na nasa post po para alam niyo na gagawin niyo bago kayo bumisita sa PCSO.

    baby joyce, hello po gusto q pong mag apply s PCSO,mayroon po aqng mlking bokol s leeg sbi po s biopsy colloid cyst daw 2x n po i2ng inoperahan taz nagbalik po uli after a month.gudto q pong ipa opera uli kso wala po kming pera at alam qng mlki ang gastod d2.paano po aq mtulongan ng iyong office s hospital bill at gamot thank you God bless

    Nag apply kmi last june for medicine assistance for my mom.mas malaki pa nagastos namin kakapabalik balik samin kesa sa binigay na gamot. We complied with all the requirements from the very first day na ng visit kami s pcso,pero talagang mabagal ang process ng pcso.ang nkakainis,kailangan lahat ng mga gamot pero sa 10 klase ng gamot na ni request nmin, 1 klase lng ang binigay kasi hindi daw available.

    Hi good day po maaring kaya ako matulungan ng pcso para maka pag pa ear reconstructive surgery ako na pingas po kc ung right ear ko due to human bite then nag hanap po ako sa internet kung saan may gumagawa nito may nkita nman po ako hospital at doctor kaso ung cause ng reconstruction nya is malaki aabutin dw ng 100k for two surgery kaso wla naman po ako ganun kalaking pera maliit lng dn ang kita ko at nagpapaaral ng dalawang anak madalas pa ay kulang ang aking kinikita pra matugunan lhat ng gastusin nmen sa pang araw araw sana matulungan ako ng pcso para maka pamuhay din ako ng normal hindi ung may tinatagong deformities sobra sobra ang stress na inabot ko sa nangyari sken nag karoon ako ng self esteem at nawalan ako ng self confidence sa sarili 10 years ago na din pro hangang ngayon sariwa pa din sken ang ngyari sana po matulungan nyo ako kung sino man ang makabasa nito maraming salamat po godbless po

      Hi Miguel, regarding your case please visit a PCSO office. Dun kasi sa guideline ng PCSO reconstructive surgery especially cosmetic in nature ay hindi sakop. Yung unique case mo ang PCSO mismo ang makakapagsabi if covered sya, or not. Kaya para ma-100% sure please go to PCSO and talk to their officers there.

    Good morning po, I have my sister who is presently confined at Cebu Velez Hospital for more than 2 weeks now, she is also a government employee but due to her ailments, she is now in her critical state that medication is sometimes be our problem dahil nasa private hospital. Gusto ko po siyang tulungan kaso nasa Mindanao ako ngayon at nasa Cebu po siya. Paano ko po matutulungan sa pamamagutan po ng programa ng PCSO?

    Last year phumingi kmi ng assistance from PCSO,hanggang ngayon d pa nmin ntatanggap ung amount n cnbi nlang marereceive nmin.Dengue stage 4 po ung anak ko sa Saint Paul hospital Tuguegarao.Pwede pa po b nming I follow up un?

    Advice lang po, try nyo po makumpleto yung mga requirements na kailangan nyo, then pumunta po kayo sa PCSO sa lugar nyo po mismo (need po kasi na kung saan kayong city or province nakatira ay dun kayo mag-a-apply), at need po talaga na makausap nyo yung employee ng PCSO na nagko-conduct ng initial interview, para malaman nyo po kung paano nila kayo matutulungan sa problema nyo.     good eve po,ako po judy son of ma. theresa baclay,tga cebu po kami nkaconfined si mama ko sa isang private hospital,tnx god ok na po sya pero hindi po makalabas dahil po sa laki ng hospital bill namin umabot po kasi ng mahigit 200 000 ea wla po kaming pambayad,,kung pwede po hihingi sana kami ng tulong sa inyo para sa aming bill,,kais mag-iisang buwan na po c mama nghintay ng tulong pra mkalabas…tnx po

      Hi Judy, based dito sa dalawang comment mo, naka-hingi ka na ng tulong sa PCSO but yung assistance na naibigay dun lang sa gamot, tama ba? Our suggestion for you would be to approach again PCSO and plead your case and request them for help again. Puede ka ding magtry approach sa LGU ng Cebu City baka may maitulong sila. We hope with you na sanay makauwi na mama mo as soon as possible.

    sa private hospital po c mama nadala kasi po yun lang ang pinakamalapit na hospital kung saan siya inatake sa puso…may mga tulong po naman kaming natanggap pero hanggang gamot lang po….sana po ay matulungan niyo po kami…tatanawin po naming malaking utang na loob sa inyo po,,,maraming salamat po…..nasa south general hospital nga po pala c mama nka confined sa cebu….salamat po ulit

      Hi Judy, kung wala pa kayong natatanggap na tulong sa PCSO, punta po kayo sa PCSO nearest your residence. Or ask po kayo sa Social Welfare officer ng hospital kasi baka yung hospital mismo meron ng PCSO desk. May mga hospital kasi na merong PCSO Coordinator na.

    Ilang weeks po ba bago ma grant yung application?? Na submit na po kasi namin yung requiremnt ang sabi e ttxt nalng daw kami.. one week na po nakakalipas wala pa rin.. 🙁

      Wala pong fix processing dates ang PCSO, ang best po na gagawin is mag follow-up po kayo kung ano na ang status ng Medical Assistance application niyo po. If possible, try to reach out dun po sa nakausap niyo nung nag apply or yung nag handle ng case niyo mismo.

    Kahit po ba may philhealth at health card eh tumutulonv pa din po ba ang pcso? Kc sobrang lake pa din po ng bill namin kahit na less na yung s health card? Kaya di po makalabas ng hospital.. sa hospital po ba ipapangalan ung cheke o sa nag lakad ng papers? Tnx

      Hi Kaycee, icheck mo if merong PCSO desk yung hospital na affiliated yung doctor ng asawa po tapos hingi ka dun ng assistance. If walang PCSO desk yung hospital, need mo pumunta talaga sa office ng PCSO para na din makuha mo requirements na kailangang isubmit para sa case ng asawa mo.

      Puede pa din po, lahat kasi ng Filipino ay puedeng humingi ng medical assistance sa PCSO. Kaso ang ward patients ang may priority assistance, pero puede pa din po humingi ng tulong.

      Hi Mark, pag next time ma encounter niyo ulit yan, mag request sa guard na if puede irefer kayo sa isang PCSO personnel mismo. Pag hindi papayag, sabihin niyo na mapipilitan kayong ireport sa hotline 8888 ang nangyari. Siguro naman papayag sila kasi kailangan ng mamamayan na mismong staff ng PCSO ang makausap.

    Hi… Ask ko lang poh kasi ung kapatid ko kelangan na kelangan na sya mshockwave ung both kidneys nya may mga bato na saka un pong tubo nya sa gitna kidney at pantog may nakabara na. Ung creatinine nya po sobrang taas na saka ung testicles nya namamaga na po. Pwede po kaya mailapit sa pcso? Walang wala po kaming pera kelangan narin maishockwave yung kapatid ko po.       Hi po, itanong niyo po sa hospital nurse if meron silang PCSO desk sa loob ng hospital then punta kayo dun. If wala punta po kayo sa pinakamalapit na PCSO, dalhin niyo po lahat ng hospitalization record. Also po, hingi din kayo ng tulong sa munisipyo or city hall niyo, baka may maiambag din po sila sa inyong mga bayarin.

    Hello po. Just want to inquire about my son’s case. Meron po sya Beta Thalassemia (major). Monthly po ang blood transfusion nya and nag iron chelation na din po sya for 9 months. Lifetime po to. The iron chelation agent is too expensive and yung monthly transfusion din po. Can we avail of medicine assistance and blood transfusion.

    Hi, tinutulungan ko po ngayon yung family ng girlfriend/fiance ko kahit konting amount lng. Hindi po nila mabayaran ang hospital bills sa SPMC, yung mother niya was diagnosed, ang nangyari, may blood clot sa veins niya apat ang nakita, then after 2 days nag run ng test ulit nalaman yung apendix sumabog at maraming gall stones sa gallbladder niya. Nag-approach sila sa Lingap sabi kailangan sa PCSO idaan bago sila dahil sobrang laki ng bills babayaran sa hospital. Pwde po ba khit hindi family member lumapit sa inyo for help para sa kanila? Also ang sabi, they need to fly to Manila and present the documents required para maka.assist po sa kanila. The problem is wala po sila funds to travel. Pwde po ba ako na lng magsubmit ng documents since andito po ako? isend na lng nila ang documents sa akin.

      Hi Edsel, puede po ibang tao ang mag process ng PCSO provided na meron kang authorization letter na original plus valid ID mo at valid ID ng pasyente, together with other docs like medical certificate, lab results, etc. Ipatanong din sa hospital, baka meron silang PCSO assistance desk sa loob mismo ng hospital para duon nalang iprocess.

    Ako po ay na Kidney Transplant noong 2003 at nag fail na nung nakaraang January(2017). Sa ngayon po ako ay nag dialysis(2x/week) ulit at nangangarap na ma kidney transplant ulit. Wala na po kaming pera pang pa opera kaya nagbabaka sakali po ako na maari nyo po akong matulungan na ma operahan.
    Sana po ako ay inyong matulungan upang maging mas maayos at madali ang buhay kong muli.

We need at least 10500 for gastroscopy, yan po estimate samin from San Juan De Dios na hindi macocover ng HMO. Ask ko din po if ano ung Certified/True Copy of the Updated Clinical Abstract? sa Doctor na po ba namin to kukunin?

    Magandang araw po sa inyong lahat na taga-pangasiwa ng PCSO, isa po sana ako sa nangangailangan ng inyong tulong na naghahangad na mabigyan ng pagkakataon.
    Isa na po ako ngayong PWD sa dahilan ng pagkaputol ng kaliwa kong paa. Ang kailangan ko po ngayon ay Ang makatayong muli sa pamamagitan ng prosthetic leg, ngunit medyo may kamahalan Ang magkaroon ng ganon..kaya sa inyo ko po naisipang lumapit dahil nabalitaan ko po na marami na kayong taong natulongan na kagaya kong isang mahirap lamang.
    Ang naging problema ko ngayon ay kung paano ako makakuha ng qoutations from 3 other suppliers. Taga mindanao lang kasi ako specific na lugar ay OZAMIS CITY…kahit isang supplier dito ay wala..kailangan ko pa na pumunta ng maynila…
    Ano po Ang dapat kong gawin..sana po ay may makapansin….salamat po ng marami…GOD BLESS….

      Sir, baka meron po kayong kakilala sa Manila or Cebu na puedeng mag request ng quote sa supplier? sana meron po kasi kailangan ng PCSO yun. Or if malapit kayo sa PCSO branch dyan sa Ozamiz, try niyo po ask directly sa kanila kung may exceptions po ba na umiiral dyan sa lugar niyo dahil walang supplier mahanap sa probinsya. Sana po makakuha kayo ng tulong agad agad sa PCSO.

    Mag ask lang po ako yung tatay ko kasi ang sabi ng doctor is yung chemo nya need na iconfine sya ng limang araw. minuminuto kasing dumudugo ilong ng tatay ko. nasoparyngeal carcinoma. hindi na po kasi kaya sa public hospital dahil po sa sitwasyong walang tigil dugo ng ilong nya. ang gusto ko po malaman is kung matutulungan po ba kami sa hospital bill ng pcso kahit po ang sitwasyon nya ay Chemoteraphy pero Confine ng 5 days. kasi 5 days po syang ichemo. thank you po sa sasagot please po help naman po.

    Gud day… Im darimar lakibul, a government employee, residence of jolo sulu, working as med tech at sulu Sanitarium hospital… Recently i was admitted at ciudad medical at zamboanga city and was diagnosed as MI or myocardial infarction.. And was advice to undergo andiogram procedure and later for andioplasty at heart center. Ive search thru net d costing of its procedure.. For andiogram it will cost around bet 50k-60k and for andioplasty 200k – 500k.. In line with this i am seeking from your good office medical assistance to at least cater the mentioned procedure. I dont have financial capabilities to pay for d procedures… Hoping this will merit your favorable consideration

I am unemployed since 2013( I suffered a stroke and unemployed since then.)I was diagnosed with chronic Atrial Fibrillation & Cardio Myopathy. I have to take maintenance medication or life. I only have my retired mother to finance my medication, but she is also taking maintenance medication on her own. Her pension is not enough for our medication and for our daily needs.

      Hi Lizette, puede ka po i-authorize ng father mo to process his application sa PCSO Medical Assistance. Make sure lang to bring all the required docs. Much better if you can call PCSO ahead and share all the docs you currently have with you if compliant na sa mga required documents. Sana ma-approve ang case ng tatay mo as soon as possible.

    I applied for a maintenance Medicine assistance last December 12, 2017, but my application was rejected because maintenance medicine assistance is NOT available.I have been e-mailing (2x) mr.     it’s a timely topic for me and i have a somehow related question that if answered would also be helpful to others seeking assistance from pcso.

    Original or Certified true copy of the medical abstract or record signes by the attending physician, that’s one of the requirement.

    Is there anybody here know how to obtain a certified true copy of a medical document? Can we have it done through a notary public? Or should it be done on the same place or facility who issued the original document?

    I hope that somebody would be able to clarify it so it woyld be more easier for other patients or patient relatives to obtain these kind of requirements.

    Regarding po sa Lupus patient pero hindi pa naman po ganun kalala yung Lupus bale monthly monitoring po siya para hindi lumala ang Lupus kaya may monthly checkup po siya pati laboratory test na ginagawa sa kanya every month at may maintenance din pong gamot na iniinum para hindi po magtrigger ang Lupus. Ask ko lamang po meron po bang assistance na maibibigay sa isang Lupus patient para po sa monthly monitoring po at sa gamot na iniinum po. Bale bilas ko po yung may sakit na Lupus may kamahalan po kasi yung maintenance na gamot at obligado pong inumin niya kasi kung hindi po makakainum lalala po yung Lupus niya, kaya po baka may maitulong po ang PCSO sa kanya kahit po sa monthly maintenance ng gamot niya.     itatanong ko lang po meron po akung mayoma at ovary cyst kailangan ko pong makabili ng gamot na halagang 20100 aang total kada isa po ay 6700 po 3 beses po gagawin ittanug ko lang po kung pwd po ba akung lumapit sa pcso para doon sa gamot ko salamat po

    Hi, Itatanong ko lang po kung pwede ko po ba iapply ng assistance ang tatay ko, naconfine po siya before mga 2016 po then na stroke na po siya, until now po kasi madami siyang maintenance na gamut na iniinom,

    Hi year 2016 naoperahan ako sa UNITED ARAB EMIRATES ABU DHABI dahil sa endemetriosis ovarian cyst at based po sa dr. ko meron nanaman po marami at 1 year na po ako wlng checkup kc po di ko po afford yung i mantain ung checkup pde po kaya ilapit.

    ask lang po , may limit po ba na bed/room rate para makahingi ng tulong sa pcso? kc ung samen ndi tinanggap nung sa pcso kc po naka isolation room kame.,. 5200 ung rate,. ang required lang daw nila is 4,500 na room rate?
