How to Apply for PCSO Assistance for Dialysis Patients – Hemo and Peritoneal

Guideline for Hemo or Peritoneal Dialysis patients seeking PCSO assistance.  The Philippine Charity Sweepstakes Office requires enrollment for these patients to avail this type of Individual Medical Assistance Program (IMAP) specifically for dialysis medicines request from their office.

The enrollment system of PCSO aims to provide financial assistance to patients with kidney disease to help patient cover their expenses of the costly medicine EPOIETIN and laboratory procedures while undergoing dialysis.

The dialysis session or treatment itself is not covered, what is covered is financial assistance to buy the drug Erythropoietin or Epoietin which is needed for dialysis sessions and lab procedures.

Apply PCSO Dialysis Assistance

The old assistance given by PCSO was on dialysis sessions itself but this was suspended and changed to the current one which is giving financial aid to buy the expensive dialysis medicine Epoietin. The reason for the changed is said to be that PCSO stopped the grant of the dialysis session aid after it learned that the Philippine Health Insurance Corporation (Philhealth) had increased the number of hemodialysis treatment coverage from 45 treatments to 90 sessions per year.

Since Philhealth has increase the number of dialysis treatment sessions it covers to 90 sessions annually, PCSO can offer more help by giving aid to buy the medicine needed for the dialysis treatment. There are some groups though who prefers the old dialysis treatment assistance itself versus assistance for purchasing the medicine/lab procedures. There is also an appeal to PCSO now about the dialysis treatment assistance, please find the link at the bottom of the post if you want to know more details about the petition.

Kindly check the details on enrollment requirements below for patients with a need for dialysis treatment as well as the amount of assistance.

Both Philhealth members and non-Philhealth members may apply to PCSO provided they meet the set qualifications and documentary requirements.

FOR PHILHEALTH MEMBERS

For Philhealth members undergoing Hemodialysis – The maximum amount of yearly assistance that PCSO can give to this group of patient is P84,000 for the drug epoietin at P14,000 per claim. There will only be a total of 6 claims in one year.

For Philhealth members undergoing Peritoneal dialsysis and who availed of the Z Package –  PCSO yearly assistance is set at P84,000 per year for medicines epoietin at P14,000 per claim. This will be a total of 6 claims per year.

For Philhealth members undergoing Peritoneal dialysis under the case rate, the maximum yearly assistance is P120,000 for dialysis solutions at P20,000 per claim. A total of 6 claims per year.

**PCSO assistance may be processed and released in six (6) equal tranches one after the other at any time upon presentation of certification of ongoing treatment from the attending physician.

FOR NON-PHILHEALTH MEMBERS

All Non-Philhealth members who wishes to enroll and avail the dialysis assistance program of PCSO are required to also submit a certification of PHIC non-membership from the attendant dialysis center. Which means you need to file a request in the dialysis center where you are undergoing treatment for a certification document that states you are not a member of Philhealth.

This is on top of all the other documentary requirements that patients need to submit for inclusion in the program.

HOW TO APPLY FOR ENROLLMENT to PCSO DIALYSIS ASSISTANCE PROGRAM

If you want to become a beneficiary of PCSO financial assistance for patients undergoing Hemodialysis or Peritoneal Dialysis, you need to apply for enrollment.

UPON ENROLLMENT / FIRST CLAIM

You must go to PCSO personally to apply for inclusion in the program. You must also bring the following documents:

  • Duly accomplished Application Form from PCSO.
  • Original Medical Abstract duly signed by the attending physician/ Nephrologist with license number.
  • Prescriptions

 Prescription if Philhealth member

*Hemodialysis & Peritoneal Dialysis Z – medicines (EPO)

*Peritoneal Dialysis case rate – PD solutions

  Prescription if Non-Philhealth member

*Peritoneal Dialysis – PD solutions

  • Official quotation from the dialysis centers / hospitals.
  • Updated laboratory results.
  • Certification of acceptance of Guarantee Letter (GL) from the dialysis center / hospital.
  • Certification of PhilHealth membership status including type of package for Peritoneal Dialysis (Z or case rate).
  • Valid ID (patient valid ID and designated representative valid ID if a representative will process succeeding releases of claims)

Your case will be evaluated by the PCSO personnel in charge together with all the required documents you’ve submitted.  However, you may need to go to PCSO office more than one time in order for you to get the application form, Guarantee Letter (GL), then you have to present the GL to the dialysis center so they can issue the certificate of acceptance, then bring the acceptance certificate back to PCSO, etc.

Once you are approved, you will also be given an index card / client-social worker agreement, keep this document safe since you need it when filing the next claims.

SUCCEEDING CLAIMS

For dialysis patients who are already enrolled and filing for succeeding claims – you or your official designated representative can file for assistance claim in your behalf in PCSO.  You or your representative needs to bring the following documents on your 2nd claim, 3rd claim, until 6th claim for the year:

  • The index card/client-social worker agreement issued upon enrollment must be presented to claim the Guarantee Letter (GL)*.
  • Certification of ongoing treatment including the last date of treatment from the Nephrologist or attending physician.
  • Original prescriptions

For Philhealth members

*Hemodialysis & Peritoneal Dialysis Z – medicines (EPO)

*Peritoneal Dialysis case rate – PD solutions

For Non-Philhealth members

*Peritoneal Dialysis – PD solutions

      iCheck niyo po if may PCSO assistance desk yung hospital, if wala need niyo po pumunta sa PCSO para asikasuhin ang requirements.

    Hi, good morning, just want to clarify something about asking for medical assistance from PCSO for dialysis case…just yesterday December 1, 2016 my brother applied for such assistance and he was given a certificate worth 7 thousand pesos…as I was reading ur posted article for this case, I noted that the maximum amount of yearly assistance is 84 thousand pesos with a total of 6 claims in one year and per claim is 14 thousand pesos…my inquiry is that he was given only 7 thousand pesos in his certification…he applied at PCSO – OZAMIS, MISAMIS OCCIDENTAL…hope u can give us some answer to this inquiry…thank u

      Hi Ann, is your brother already enrolled in the PCSO IMAP Program for dialysis patient or was the certificate represents only a regular medical financial assistance? If he is not enrolled, it would be best for him to go there again and bring all the requirements as stated on this post. While there, have him ask directly the PCSO personnel his status on IMAP enrollment as a dialysis patient.

    Hi! I already enrolled in the assistance program last October 28 and was told to wait for a text on when to claim the guarantee letter. Until now, I haven’t received any text. My question is – is it ok to call PCSO and ask for an update/followup? What numbers should I call? Thanks very much

      Hi Marie, follow-up is okay, but you should contact the PCSO center where you went to. Try to Google the number since we don’t know which PCSO branch handled your case and thus can’t give you possible number to call. Hope makuha mo na agad ang guarantee letter.

      Hindi po, filing for request of PCSO assistance can be process by an authorized representative, mas maigi na kamag-anak yung iauthorized nung patient.

    hello good day mam/sir ako po gregorio panuncialman nais kopo sana humingi ng tulong ng PCSO dahil po wala po kami sapat pang laboatory matagal na po ako nagkakasakit kaya hndi napo ako nakapag trbaho noong nag pa chic up po ako sa doktor sa visayas community hospital nagbibigay siya ng request na kailangan ko raw mag pa ct scan of the brain, teadmill test, ultrasound of the neck,, hanggang ngayun hindi pa ako nagpaloboraory dahil wala talaga kami sapat na budjet umiinum nalang po ako ng herbal plants ngunit unti unti napo ako nanghihina kaya ninanais kopo na mag sulat para humingi po sa inyo ng tulong para makapg pa chck up para po sa aking karamdaman maraming salamat po sa p[ag basa

      Hi Gregorio, requirements are in the post above, if the illness is not included above, it is best to go visit a PSCO branch. Or if may government hospital sa area mo, puntahan mo, usually may PCSO assistance desk ang government hospitals. Itanong mo ano requirements nila for your unique case. If di po ninyo kaya magprocess na kayo mismo kasi nanghihina kayo, puede po kayo mag-authorize ng kamag-anak niyo. Sana makakuha po agad kayo ng tulong at gumaling soonest.

    Magandang araw po. Nais ko po sanang mag-avail ng financial assistance from PCSO para sa asawa ko na may-sakit na diabetes at kailangan na kailangan niya ng regular na supply ng Epoietin. Isa po sa nakita ko na requirement ay ang kompletong pag-fill up ng IMAP Application Form. Sinusubok ko po magdownload ng form mula sa PCSO website pero lagi po naka-down ito at “under maintenance” daw. Saan po bang website o lugar kung saan maaaring makakuha ng form na pwede i-download at i-print ko na lang? At dahil hindi na po nakakakita ang asawa ko sa dahilang malabo na pareho ang mga mata niya, pwede po ba na ako na lamang na asawa niya ang mag-fill up nito? Maraming salamat po sa inyong magiging tugon.

    Binigyan po ako ng Guarantee Letter ng PCSO Bulacan ng P25 T para sa Dialysis treatment ito lang Sept 2018. Ang tanong ko po ay sabi ng Meycauyan Doctors Hospital ibabawas
    daw nila yong full amount ng charge sa Dialysis na hindi pwedeng I discount sa senior citizen. Ang dating charge sa akin ay P2250 discounted na sa senior. Ngayon P3250 na raw ang charge per session at di na pwedeng ma discount sa senior. Tama po ba ito?

    Hello. Good day. Yun po ba 84,000 per year is applicable only if a dialysis patient is a philhealth member or pwde din po sa philhealth dependent.

    Bukod po sa libreng inject, may maitutulong po ba ang PCSO kapag naubos na po ang 90 days sa philhealth?
    Sa ngayon po kasi hindi na po makapagdialysis ang tatay ko at hindi ko na po alam kung saang ahensya po ako lalapit.. 🙁
